Mă numesc Ioana-Alina Cristea, conferențiar universitar la Departamentul de Psihologie Clinică și Psihoterapie, Facultatea de Psihologie și Științele Educației, Universitatea Babeș-Bolyai. Am finalizat studiile în Psihologie, urmate de un masterat în Psihologie Clinică, Consiliere Psihologică și Psihoterapie la aceeași instituție. Ulterior, am finalizat în 2012 studiile doctorale în cadrul Școlii Doctorale „Psihodiagnostic şi intervenţii psihologice validate știinţific” (sub coordonarea științifică a d-lui prof. univ. dr. Daniel David). Activitatea mea științifică s-a focalizat pe implementarea paradigmei „evidence-based medicine” (medicina bazată pe dovezi) în domeniul psihoterapiei și al psihologiei clinice, cu o focalizare atât pe dovezile legate de eficiența unor intervenții psihoterapeutice în a produce rezultate clinic relevante, cât și de susținerea empirică pentru mecanismele care ar putea fundamenta aceste schimbări terapeutice. Aceasta este de altfel una din temele principale de cercetare ale Şcolii clujene de psihologie clinică şi psihoterapie (atât a Școlii Doctorale, cât și a Departamentului de Psihologie Clinică și Psihoterapie, cu precădere a Institutului Internațional de Studii Avansate de Psihoterapie și Sănătate Mentală Aplicată care concentrează activitatea de cercetare a departamentului). Abordarea mea a fost una critică și majoritatea studiilor mele au scos în evidență probleme sistematice și distorsiuni cu potențial de a influența considerabil rezultatele raportate. În ultimii ani, activitatea mea științifică s-a extins mai larg la domeniului „meta-cercetării” (cercetare despre cercetare), care încearcă să o imagine de perspectivă asupra modului în care e proiectată, condusă, raportată și utilizată cercetarea științifică în general, investigând distorsiuni recurente, erori metodologice sistematice, factori externi (ex., rolul sponsorului) care pot avea o influență nefastă, precum și explorarea unor posibile soluții. Pe această direcție, în prezent sunt într-un stagiu de cercetare la Universitatea Stanford, SUA, în calitate de Fulbright Visiting Senior Scholar, unde colaborez cu unul dintre cei mai proeminenţi cercetători din acest domeniu, profesorul John Ioannidis.

My name is Ioana-Alina Cristea and I am Associate Professor at the Department of Clinical Psychology and Psychotherapy, Faculty of Psychology and Educational Sciences, Babes-Bolyai University. I finalized my undergraduate studies in Psychology, followed by a Master in Clinical Psychology, Psychological Counselling and Psychotherapy at the same institution. I subsequently completed my doctoral studies in 2012 in the Doctoral School “Evidence-based psychological evaluation and interventions” (under the scientific coordination of professor, Ph.D. Daniel David). My scientific activity has been focused on implementing the “evidence-based medicine” paradigm in the fields of psychotherapy and clinical psychology, with a concentration on both the evidence supporting the efficacy of psychotherapeutic interventions in producing clinically relevant outcomes, as well as on the empirical support for the mechanisms that could fundament these therapeutic changes. In fact, this is one of the mean research topics of the Doctoral School, as well as of the Department of Clinical Psychology and Psychotherapy, particularly the International Institute for the Advanced Studies of Psychotherapy and Applied Mental Health, concentrating the department’s research activity. My approach has been a critical one and most of my studies have revealed systematic problems and biases susceptible of considerably influencing reported findings. Over the past years, my scientific activity has extended more generally to the field of “meta-research” (research about research), which attempts to give a bird’s eye view over the way in which scientific research is designed, conducted, reported and utilized. It sets to investigate recurrent biases, systematic methodological errors, external factors (e.g., the role of the sponsor) that could have an untoward influence, and it also explores potential solutions. On this direction, I am presently doing a research fellowship as a Fulbright Visiting Senior Scholar at Stanford University, USA, where I am collaborating with one of the most prominent researchers in this field, Professor, Ph.D. John Ioannidis.

De ce aţi ales UBB? (Why BBU?)

Răspuns/Answer:

Am început să lucrez cu d-l prof. David din timpul studiilor masterale și am fost în situația foarte privilegiată de a-mi regăsi interesele de cercetare în temele abordate de școala doctorală, departament și institut. Mediul a fost în permanență unul stimulant din punct de vedere intelectual și unde am putut să îmi urmăresc interesele de cercetare, beneficiind în același timp de susținere și de un input foarte prețios, care m-a ajutat în clarificarea și redefinirea ideilor. Mai mult, am avut și libertatea de a explora alte medii academice, pe perioada studiilor doctorale și ulterior având diverse stagii de cercetare la Universitatea Pisa, Universitatea Padova, ambele în Italia, și în prezent la Universitatea Stanford, SUA. Ideea de a rămâne în UBB cred că a fost una organică, unul din acele lucruri care a venit natural o dată cu tranziția de la student la doctorand și apoi cadru didactic și membru al departamentului și nu mi-am pus cred, niciodată, serios întrebarea „de ce?”. Din punct de vedere al avansării în carieră, consider de asemenea că am avut o poziție foarte privilegiată, beneficiind tot timpul de un tratament echitabil, corect, transparent și de susținere și toate resursele pe care departamentul a putut să mi le ofere.

I started working with professor, Ph.D. David over my masteral studies and I was in the priviledged situation of encountering my research interests in the topics of interest for the doctoral school, department and institute. The environment was permanently intelectually stimulating and I could pursue my research interests, while at the same time benefiting from support and valuable input, which helped me clarify and redefine ideas. Moreover, I had the freedom to explore other academic environments, by having various research fellowships during my doctoral studies and after, at the universities of Pisa and Padova, both in Italy, and presently at Stanford University, the USA. The notion of remaining in BBU was I think an organic process, one of those things that came naturally alongside my transition from student to Ph.D. student and then academic staff and member of the department, and I really never questioned why. From the point of view of career promotion, I also believe I had a very privileged position, having always benefited from an equitable, correct, and transparent treatment, as well as support and all the resources the department could offer me.

Puteţi să ne faceţi o scurtă prezentare a realizării academice care a prilejuit acest interviu? (A brief presentation of the academic achievement)?

Am publicat recent (februarie și martie 2017) ca autor principal (prim autor și autor corespondent) două articole în reviste științifice aflate în top 3 Web of Science din domeniile Psihologie și respectiv Psihiatrie. Prima este o contribuție în revista Psychological Bulletin, al doilea jurnal științific ca factor de impact (14.83) în domeniul Psihologie: Cristea, I.A., Stefan, S., Karyotaki, E., David, D., Hollon, S., Cuijpers, P. (2017). The effects of cognitive behavioral therapy are not systematically falling: a revision of Johnsen & Friborg (2015). http://dx.doi.org/10.1037/bul0000062. În acest articol, împreună cu colegi de la Departamentul de Psihologie Clinică și Psihoterapie (lect. univ. dr. Simona Ștefan și prof. univ. dr. Daniel David), precum și colaboratori de la Universitatea VU, Amsterdam, am refăcut și re-analizat o meta-analiză foarte influentă, extrem de discutată și mediatizată, care părea să demonstreze un aparent declin al eficienței psihoterapiei cognitiv-comportamentale pentru depresie. Demersul nostru a evidențiat erori fundamentale în modul în care a fost realizată această meta-analiză și a demonstrat că o dată cu corectarea lor, declinul evidențiat nu mai exista, fiind cel mai probabil un rezultat accidental.  Al doilea articol a fost o meta-analiză în revista JAMA Psychiatry (fosta Archives of General Psychiatry), a doua ca factor de impact (14.41) în domeniul Psihiatrie: Cristea, I., Gentili, C., Coteț, C., Palomba, D., Barbui, C., & Cuijpers, P. (2017) Efficacy of psychotherapies for Borderline Personality Disorder: A systematic reviews and meta-analysis 10.1001/jamapsychiatry.2016.4287. Acest articol a implicat colegi din Departament (cercetător dr. Carmen Cotet), precum și colaborarea cu colegi de la Universitățile Padova și Verona, Italia, și de la Universitatea VU Amsterdam, Olanda. Este prima meta-analiză comprehensivă a intervențiilor psihoterapeutice pentru tulburarea de personalitate borderline, iar jurnalul a publicat-o însoțită de un comentariu editorial de la unul dintre cei mai proeminenți cercetători în domeniu (prof. Peter Fonagy), un tratament rezervat contribuțiilor considerate foarte relevante. Rezultatele au pus în evidență o eficiență consistentă dar destul de modestă a abordărilor psihoterapeutice în general. Însă au arătat și o eficiență redusă mai ales la intervale mai lungi de timp post-intervenție (follow-up), și au subliniat o serie de probleme metodologice în modul în care au fost conduse studiile clinice controlate, precum și influența acestor probleme asupra rezultatelor. De asemenea, am evidențiat efecte interesante și cu potențiale aplicații practice majore legate de structurarea grupului de control, de exemplu faptul că un grup de control care a urmat un tratament manualizat a avut efecte similare cu grupul de pacienți care primea tratamentul experimental specific creat pentru această tulburare.

I recently published (February and March 2017), as principal author (first and corresponding author) two papers in scientific journals situation in the top 3 in the Web of Science domains of Psychology and, respectively, Psychiatry. The first one is a contribution in Psychological Bulletin, the second journal as impact factor (14.83) in the Psychology domain: Cristea, I.A., Stefan, S., Karyotaki, E., David, D., Hollon, S., Cuijpers, P. (2017). The effects of cognitive behavioral therapy are not systematically falling: a revision of Johnsen & Friborg (2015). In this article, together with colleagues from the Department of Clinical Psychology and Psychotherapy (assist. prof. Simona Stefan and prof. Daniel David), as well collaborators from VU University Amsterdam, we revised and re-analysed a very influential meta-analysis, which had been extremely discussed and publicized and which seemed to demonstrate an apparent decline of the efficacy of cognitive behavioral therapy for depression. Our approach evidenced fundamental errors in the way in which this meta-analysis had been conducted and demonstrated that, following their correction, the evidenced declined no longer existed, being most likely a spurious finding. The second article was a meta-analysis in the journal JAMA Psychiatry (formerly Archives of General Psychiatry), the second as as impact factor (14.41) in the domain Psychiatry: Cristea, I., Gentili, C., Coteț, C., Palomba, D., Barbui, C., & Cuijpers, P. (2017) Efficacy of psychotherapies for Borderline Personality Disorder: A systematic reviews and meta-analysis 10.1001/jamapsychiatry.2016.4287. This paper involved colleagues from the department (Dr. Carmen Cotet), as well as the collaboration with colleagues from the Universities of Padova and Verona, Italy, as well as VU University, Amsterdam, the Netherlands. It represents the first comprehensive meta-analysis on psychotherapeutic interventions for borderline personality. The journal published it accompanied by an editorial comment from one of the most prominent researchers in the field (prof. Peter Fonagy), a treatment reserved for contributions considered highly relevant. Our results showed a consistent but modest efficacy for psychotherapies in general. But they also showed a reduced efficacy at longer time spans post-intervention (follow-up), and underscored a series of methodological problems in the way randomized clinical trials had been conducted, as well as the influence these problems had on outcomes. Moreover, we evidenced interesting and potentially major practically relevant applications related to the structuring of the control group, such as the finding that a control group receiving a manualized treatment had similar effects as the group of pacients receiving the experimental treatment specifically designed for this disorder.

Care sunt planurile academice de viitor? (Future academic plans?)

Planurile academice viitoare vizează extinderea și aprofundarea studiilor în domeniul meta-cercetării, cu o focalizare predominantă pe psihologie clinică și psihologie în general. Mai precis, intenționez să îmi extind cercetările asupra studiilor clinice controlate pentru tratamentul tulburărilor mentale în general, incluzând atât psihoterapie cât și farmacoterapie, și mai ales asupra problemelor de proiectare, implementare și raportare ale acestor studii. În strânsă legătură, mă interesează investigarea distorsiunilor sistematice induse de factori externi studiilor clinice, cum ar fi sponsorizarea acestora de către industria farmaceutică sau alte entități pro-profit, sau expertiza și conflictele de interese ale celor care conduc aceste studii. Alte domenii de interes corelate vizează utilizarea unor tehnici meta-analitice recente (meta-analiza pe datele pacienților individuali) pentru a testa posibilitatea personalizării tratamentului tulburărilor mentale. Am în desfășurare un grant Tinere Echipe legat de aplicarea acestei tehnici pentru depresie și am depus o aplicație similară și pentru tulburările de anxietate.

Pe perioada de Fulbright Visiting Scholar la Universitatea Stanford, am demarat mai multe proiecte de meta-cercetare, aflate în diverse stadii de desfășurare. Acestea vizează atât studiile clinice controlate din domeniul sănătății mentale, de exemplu un proiect legat de integritatea raportării meta-analizelor asupra medicamentelor antipsihotice, precum și domeniul mai larg al posibilelor mecanisme cauzale ale tulburărilor mentale și implicit posibilele mecanisme ale schimbării pentru tratamentele acestora. De asemenea aș dori să îmi întăresc și să îmi extind rețeaua de colaborări internaționale și să cresc impactul mai general al cercetărilor mele, la nivel comunitar mai larg, dincolo de publicul specializat al jurnalelor științifice. La nivel practic, aș dori să reiau aplicația pentru un ERC Starting Grant, de această dată cu Universitatea Babeș-Bolyai, dat fiind că în urma eforturilor susținute cu precădere de universitatea noastră, a fost acordată o excepție pentru România, ceea ce permite organizarea aplicației și a eventualei implementări a grantului în același mod ca la orice altă universitate din Europa.

My future academic plans envisage the extension and deepening of my studies in the field of meta-research, with a predominant focus on clinical psychology and psychology in general. More precisely, I intend to extend my research on randomized controlled trials for the treatment of mental disorders in general, including both psychotherapy and pharmacotherapy, and specifically the problems in the design, implementation and reporting of these studies. In strong connection to this, I am interested in investigating the systematic biases induced by factors extraneous to clinical studies, such as sponsorship from the pharmaceutical industry or other for-profit entities, or the expertise and conflicts of interes of the individuals conducting these studies. Other related interests encompass the use of recent meta-analytical techniques (individual patient data meta-analysis) to explore the possibility of personalizing mental health treatment. I am presently coordinating a Young Research Teams grant employing this technique for depression and I have submitted a similar application for anxiety disorders.

Over the period of my Fulbright Visiting Scholar grant at Stanford University, I have embarked on several meta-research projects, presently in different stages of progression. These concentrate on randomized clinical trials in the field of mental health, such as for instance a project related to the integrity of reporting meta-analyses of anti-psychotic drugs. Another project involves the larger field of possible ethiological mechanisms of mental disorders and, connected to these, the possible mechanisms of change underlying their treatments. Also, I would like to strengthen and extend my network of international collaborations and to increase the more general impact of my research, at a larger community level, beyond the specialized public of scientific journals. On a practical level, I would like to take up my application for an ERC Starting Grant, this time with Babes-Bolyai University, since due to sustained efforts particularly stemming from our University, Romania has been granted an exception that affords organizing the application and the eventual implementation of the grant in the same way as in any European university.

Un scurt mesaj către comunitatea UBB? (A short message to the BBU academic community?)

Cred că integritatea și spiritul critic sunt cele mai importante valori pentru un cercetător și sper ca acestea să definească în timp comunitatea UBB, alături desigur de obiectivele legate de rezultate de excelență. De asemenea sper că modelul de promovare și colaborare de care am beneficiat eu în cadrul școlii doctorale și al departamentului să fie cel comun în cadrul UBB.

I believe integrity and a critical spirit are the most important values for a researcher and I hope these will in time be the mark of the BBU community, alongside with the objectives related to excellence. Moreover, I hope that the promotion and collaboration model I benefited from in the doctoral school and department will become the common one in BBU.
